  • What are the office hours?



    Office hours are:

    Monday- 8:30 AM to 4:30 PM

    Tuesday-12:00 PM to 6 PM

    Wednesday -12:00 PM to 6 PM

    Thursday- 8:30 AM to 6 PM

     Friday- 8:30 AM to 4 PM  
 

  • How do I schedule an appointment?



    Appointments are scheduled with the receptionist.  Please call us during normal office hours.

  • Is the office wheel chair accessible?



    Yes, the parking lot and office are wheelchair accessible. 

  • What insurance plans do you accept?

    We accept Medicare, certain types of Medicare Advantage plans, and most major insurance plans. Contact us to verify if we participate with your insurance plan.

  • If I am a first-time patient, what should I bring with me or have sent to the offic prior to my visit?e?


    Please bring your insurance card(s), photo ID, and/or driver’s license. You may bring any medical records or you can complete the forms needed to release any information to our office.  You have the  option to download and complete new patient forms from this website prior to your visit.

  • Are insurance co-pays due at the time of my visit?

    Yes,  please note that your co-pay is due at the time of your office visits. 

  • How can I access my medical records?



    You can access your medical records through Bergen Internal Medicine’s Electronic  Patient  Portal System.  You will receive information regarding our portal at the time of your office visit.

  • What diagnostic laboratories do you utilize in the office?

    We draw blood in our office and transport it to Labcorp and Holy Name Medical Center.  If your insurance requests the use of a different lab for blood testing, we will give you a prescription for  the appropriate lab center.  The patient is responsible for knowing which laboratory is accepted by their insurance company.


  • What health services does the office provide?


    We provide annual, pre-operative, sick, and wellness exams. We also conduct post-diagnostic test reviews and offer an educational plan of care.  Other exams include CDL (Commercial Driver’s License), pre-employment, and school physicals. We administer flu vaccines, pneumonia vaccines, Tdap (tetanus, diphtheria, and pertussis) vaccines, and PPD testing. 

  • How do I renew my medications?

    Non-controlled substance medications with refills remaining can be requested by simply calling your pharmacy or contacting your mail-order prescription service for the refills. If no refills remain, have your pharmacist electronically contact our office, and we will respond within 48 hours, except on weekends and holidays. We will notify you if the doctor requires an office visit before renewing the medication.


    If the medication falls into the following categories as established by the State of New York and New Jersey, it requires a mandatory visit. It means an appointment is necessary before we can renew your prescriptions. 

    These medications include the following types of drugs: 

    • Narcotics (example) Percocet , Vicoden, Fioricet Dilaudid, and Fentanyl Patches 

    • Anti-Anxiety (examples)-Valium, Xanax, Ativan Klonopin 

    • Sleep Aides (examples) Ambien,  Restoril 

    • Tramadol 

    • Lyrica 

    • Amphetamines 


  • Does the office perform "pre-authorization" for testing or medications?


    No, we do not provide pre-authorizations. Pre-authorizations are performed by the medical testing facility.  Alternatively,  medications may be an option. Please discuss this with your physician at the time of your visit. 

  • Is it necessary for me to come in for an antibiotic prescription renewal?


    Yes, because we need to reassess your condition. If your physician determines that an active infection exists, then the appropriate antibiotic medication can be prescribed.

  • What vaccines are offered by Bergen Internal Medicine?

    We offer Influenza,  Pneumonia, and Tdap (tetanus, diphtheria, pertussis) vaccines.
 We do not administer  COVID-19 vaccines. Visit our COVID-19 page for federal and local resources.

  • Do I have to fast in advance for an office visit?

    Routine checkups, annual visits, and visits to monitor blood work, such as cholesterol and glucose all require fasting.  Elderly patients are exempt from fasting for safety reasons and diabetic patients who have a history of hypoglycemia when fasting.

  • Where can I go for outpatient radiographic testing?



    We recommend that you visit any facility that is convenient and reliable.  However, if your insurance benefits require "pre-certification", any facility you choose is required to fulfill the "pre-certification" process. Our office does not perform pre-certifications.

 

  • How do I contact the doctor for an emergency after the office closes?



    If it is a true emergency, CALL 911 and go to the closest Emergency Room.  After office hours, you can contact our physicians for emergent matters by calling our main number. Our answering service will reach out to your physician so they can return your call. Medication renewals are handled ONLY during office hours.

  • If the doctor does not respond within a half hour, what should you do?

    If necessary, please call the main telephone number AGAIN and alert the answering service operator to please page your doctor again.  Our goal is to espond as soon as possible! However,  we may be restricted by technology/ and or our schedule. We appreciate your patience.

  • When should medication requests be made?



    All medication refills MUST be handled during regular office hours.

 

  • What methods of payment do we accept?



    Copays are due at the time of your visit.  We prefer checks or cash.  We also accept MasterCard and Visa credit cards.  Payment is expected at the time of the office visit. 

  • What steps should I take if I have an emergency?

    DIAL 911. Please have a family member or significant other notify Dr. Gallo or Dr. Hernandez.

 

  • If I go to the emergency room, what do I tell the Emergency Room staff?

    Tell the Emergency Room staff that you are a patient of Dr. Gallo and/or Dr. Hernandez and to please notify them of your ER visit.
