How many doctors work in the office?
Two physicians work in our office.
What are the ages of patients we treat?
We treat patients 18 years and older.
Which hospitals or health care facilities are affiliated with the doctors ?
Dr. Gallo and Dr. Hernandez are affiliated with Holy Name Medical Center in Teaneck, New Jersey, Care One of Teaneck Rehabilitation Center, Teaneck, New Jersey, and Dunroven /Care One of Cresskill Rehabilitation Center, Cresskill, New Jersey.
What are the office hours?
Office hours are Monday 8:30 AM to 4 PM, Tuesday 12:00 PM to 6 PM, Wednesday 12 PM to 6 PM*, Thursday 8:30 AM to 6 PM, and Friday 8:30 AM to 4 PM. * 1st Wednesday of the month 8:30 AM -4:00 PM
How do I schedule an appointment?
Appointments are scheduled with the receptionist. Please call during office hours.
What insurance plans do you accept?
We accept Medicare, Oxford/ United, Cigna, certain Blue Cross/ Blue Shield plans, Holy Name QualCare and others. Please contact the office to verify our participation with your insurance plan.
Does the patient’s insurance affect testing sites?
We will review with you which companies we utilize for laboratory and radiologic testing. If you have information that differs from what we have identified, please communicate with us. The patient is responsible for knowing which laboratory their insurance accepts.
Is the office wheel chair accessible?
Yes. If you are having trouble parking, the office is to be notified.
If I am a first-time patient, what should I bring with me or have sent to the office?
Please bring your insurance card(s), photo ID and/or driver’s license along with your medical records. You can call ahead to have your records sent from your previous physician, hospital or clinic. If your medical records are not available at the time of your first visit, please arrange to have them sent soon thereafter. You may download paperwork to fill out prior to visit.
What does the office offer for the patient?
We offer well and sick visits, pre-operative, annual and well exams. Other exams include: CDL (Commercial Driver’s License, pre-employment and school physicals. We also perform Tuberculosis testing.
During an office visit, what do you assess during a physical examination?
We assess your vital signs, which include: your weight, height, blood pressure, heart rate, and temperature. We examine your heart, lungs abdomen, circulation, skin, muscles and joints. An exam will be tailored to your specific condition.
Why are "mandatory visits " and why is an appointment necessary to refill or fill medications?
If the medications fall into the following categories as established by the State of New York and New Jersey then it is classified as a "mandatory visit" A mandatory visit means that an appointment is necessary. These medications include the following types of drugs:

Can my pharmacist call for medication refills?
Yes, if you are a current patient and the medication has been an established, long term treatment and does not fall in the category of mandatory visits, then a simple phone call to your pharmacist or mail away drug plan is all that would be necessary. The pharmacist will contact our office electronically. Please allow for a 24 hour response time to fulfill your request. Closed Saturday and Sunday.
Does the office perform "pre-authorization" for testing or medications?
No. "Pre-authorizations" are to be performed by the facility performing the test and alternative medications will be chosen for medications needing "pre-authorization".
Is it necessary for me to come in for an antibiotic prescription renewal?
Yes, because we have to reassess your condition and determine if there is an active infection or consider a drug resistant organism and change antibiotics.
What vaccines are offered by Bergen Internal Medicine?
We offer Influenza and Pneumonia vaccine, Td (tetanus diphtheria), and Tdap (tetanus, diphtheria, pertussis).
Can I access my medical records from the internet?
Yes, our office offers internet access through the Electronic Portal System. Contact us for more information.
How do I access my medical records or test results?
Call the office to access your records.
Do I have to fast in advance for an office visit?
Routine checkups, annual visits and visits to monitor blood work, such as cholesterol and glucose, require fasting.
Which patients do not have to fast?
Elderly patients and individuals whom become hypoglycemic (low sugar) do not have to fast.
How do the doctors determine what blood testing is performed?
The type of testing depends on your symptom complex and recommended screenings.
Where can I go for outpatient radiographic testing?
We recommend any facility that is convenient and reliable. However, if your insurance benefits require "pre-certification", any facility you choose is required to fulfill the "pre-certification" process. Our office does not perform the pre-certification.
What happens after hours? How do I contact the doctor?
After office hours, we, the physicians, can be contacted for EMERGENCIES by calling the main number which is transferred to our answering service. We will then be contacted by our answering service.
If the doctor does not respond within a half hour, what should you do? Please call the main telephone number AGAIN and have our answering service page us again. Our concern is to respond as soon as possible! We are restricted by technology and our schedule. We are grateful for your patience.
When should medication requests be made?
All medication refills MUST be handled during regular office hours.
Do the doctors make home visits?
Yes, we make home visits for our established patients depending on the location, the doctors’ schedules, and the type of insurance and/or payment.
What payments do we accept?
We accept MasterCard and Visa credit cards, checks and cash. Payment is expected at the time of the office visit.
What do I do if I have an emergency?
DIAL 911. Please have family of significant other notify Dr. Gallo or Dr. Hernandez.
If I go to the emergency room, what do I tell the staff?
Tell the ER staff that you are a patient of Dr. Gallo and/or Dr. Hernandez and to please notify your doctors.
